To begin, please download and complete the following attachments and provide a copy of the front and back of their insurance card, along with their ASD or related diagnostic report (usually an ADOS), and finally an updated referral dated no more than one year ago. Once you filled out all necessary documents, please send them to the email provided above, and a request to conduct the initial assessment with your insurance will be submitted.
